Pay: $31,200 - $61,000 a year

Overview: As a Case Manager, you will be expected to function as an active Team Member to provide case management services and treatment based services to consumers with mental illness, developmental disability and/or traumatic brain injury. Duties include one on one interaction with consumers and related activities including coordinating relevant consumer specific supports, services, housing, social networks, appointments, PCP planning, utilizing natural/community supports, maximizing income/benefits and plans of service.

Qualifications: A minimum of a Master's degree in a human services field (i.e. psychology, social work, etc.)

    Major Responsibilities

  • Serve as an advocate to those catastrophically injured in an auto-accident.
  • Use knowledge of the auto laws in Michigan and of the healthcare system to ensure appropriate care is sought out and received by clients.
  • Create a plan to rehabilitate clients.
  • Attend physician visits and speak on behalf of clients.
  • Coordinate with clients, lawyers, providers, transportation, insurance adjusters, and family members.

Nurse Care Manager -- Feinberg Consulting

A fast growing Independent Care Coordination and Health Advocacy firm is seeking a Registered Nurse with strong interpersonal skills to provide care management for West Bloomfield, MI and surrounding areas. The position entails coordination of care including but not limited to: catastrophic care management, mental health and substance abuse.

    Major Responsibilities

  • Advocate, educate, and coordinate with interdisciplinary team to maximize client outcomes.
  • Perform evaluation of client and develop treatment plan.
  • Identify client centered goals.
  • Schedule and attend doctor appointments.
  • Coordinate therapies, home care provider, transportation, DME, and other needs as described in plan of care.
  • Daily documentation through billing log of all client activity and submission monthly progress reports.
