Pay: $31,200 - $61,000 a year

Overview: As a Case Manager, you will be expected to function as an active Team Member to provide case management services and treatment based services to consumers with mental illness, developmental disability and/or traumatic brain injury. Duties include one on one interaction with consumers and related activities including coordinating relevant consumer specific supports, services, housing, social networks, appointments, PCP planning, utilizing natural/community supports, maximizing income/benefits and plans of service.

Qualifications: A minimum of a Master's degree in a human services field (i.e. psychology, social work, etc.)

Job Responsibilities:
Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
Handle case assignments, draft service plans, review case progress and determine case closure.
Help clients achieve wellness and autonomy.
Facilitate multiple care aspects (case coordination, information sharing, etc.).
Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
Develop effective working relations and cooperate with medical team throughout the entire case management process.
Take the extra mile and interact with Clients to keep track of their progress and to ensure satisfaction.
Record cases information, complete accurately all necessary forms and produce reports in a timely manner
Promote quality and cost-effective interventions and outcomes.
Assess and address motivational and psychosocial issues.
Adhere to professional standards as outlined by protocols, rules and regulations.


Requirements
Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job
Excellent knowledge of case management principles, healthcare management and reimbursement
Previous experience with psychological aspects of care
Effective communication skills
Excellent organizational and time management skills
Familiarity with professional and technical emerging knowledge
Problem-solving skills and ability to multi-task
Compassionate with teamwork skills
Current license, certification or registration
BS degree in related healthcare field
Computer skills for report writing. MS office a must
Attend company training and meetings
Multi-language a plus (Arabic or Spanish) etc.

Nurse Care Manager -- Feinberg Consulting

A fast growing Independent Care Coordination and Health Advocacy firm is seeking a Registered Nurse with strong interpersonal skills to provide care management for West Bloomfield, MI and surrounding areas. The position entails coordination of care including but not limited to: catastrophic care management, mental health and substance abuse.

    Major Responsibilities

  • Advocate, educate, and coordinate with interdisciplinary team to maximize client outcomes.
  • Perform evaluation of client and develop treatment plan.
  • Identify client centered goals.
  • Schedule and attend doctor appointments.
  • Coordinate therapies, home care provider, transportation, DME, and other needs as described in plan of care.
  • Daily documentation through billing log of all client activity and submission monthly progress reports.
